At this point, the medical community does not have a clear answer. The predisposition to developing ankylosing spondylitis does seem to run in families (statistically, if you have family members with AS, you are more likely to have AS yourself--bear in mind that only one percent of the general population has ankylosing spondylitis). And the gene marker HLA-B27 is found in an overwhelming percentage of patients with AS. However, not every individual with this gene develops ankylosing spondylitis. Interestingly enough, it has been speculated that an external environmental factor, perhaps bacteriological, may act as a trigger for ankylosing spondylitis in individuals who are predisposed to developing the condition. This is reminiscent of discussion regarding multiple sclerosis, another autoimmune disorder, for which the speculation has been that there may be a viral triggering factor involved.
