SOCIAL SECURITY DISABILITY NOTICES

The purpose of this section is to show examples of the various types of notices that a disability claimant might receive (from either Disability Determination Services, the Office of Hearings and Appeals, the Appeals Council, or a Social Security DO, or district office) while their case is being processed. These examples are drawn from correspondence generated by various hearings offices, social security offices, and the DDS in the state of North Carolina. Please bear in mind that correspondence received from analogous units in other states may look somewhat different. Additionally, please note: since notices can be as long as 20+ pages, some of these examples only include the first page of a document.

Examples of DDS, OHA, and Social Security Office correspondence

1. - Example of a DDS 10 day call-in notice

2. - DDS call-in letter that mentions the need for a future CE (this is not the CE appointment notice)

3. - Social Security Administration Notice of Award

4. - Office of Hearings and Appeals hearing appointment notice

5. - Appeals Council denial of request for review

6. - Office of Hearings and Appeals Fully Favorable Notice

7. - Office of Hearings and Appeals Unfavorable Notice
