applying for disability

PROVIDE YOUR DISABILITY LAWYER ALL THE INFORMATION YOU HAVE

This information may help claimants with representation, as well as claimants who are not represented by an attorney or non attorney representative. Understanding how the Social Security Disability system works can make the difference between winning or not winning the continuing ssd or ssi disability benefits and backpay to which a person is entitled.

Have all your information ready when you first contact your rep

When you contact your Disability lawyer or non attorney claimant's representative for the first time, try to provide as much information about your claim as possible. This includes the level your claim is currently at (is it pending a decision, were you recently denied, are you waiting on a hearing date?) and the names and addresses of your doctors, as well.

It's amazing that many people are not prepared when they do this, but a lack of preparedness can potentially affect your case.

You need to have this information available when you first contact your rep.
  1. The last action on your case (and the date of this last case action).

  2. If you appealed, when this was done.

  3. Your medical history (essentially, your diagnosed conditons).

  4. A list of your doctors and hospitals complete with addresses and phone numbers (some reps may ask you about your prescriptions and job history but usually this information is not needed until prior to a hearing).
