WHAT SHOULD I TAKE WITH ME WHEN I GO TO FILE FOR DISABILITY ?





When you file for Social Security disability, or for SSI disability benefits, you have to provide non-disability information as well as medical information during your disability interview.

Social Security requires information about your dependent children, workman’s compensation, military service, and also marriage information (past marriages and present marriage). Social Security may also require that you provide specific documentation concerning your birth, military service, and marriage.

You should fill out any paperwork that was mailed to you within your Social Security disability application packet. Your social Security disability interview will be much smoother if you have all of your information, as well as documentation of your birth, military service, and workman’s compensation (assuming, of course, you have a workman's comp claim pending).

Additionally, it may be a good idea, prior to your disability interview date, to compile a list of all your medical treatment sources, including the names of your treating physicians and the names of all the clinics and hospitals where you have received medical treatment (be sure to include your dates of treatment).

By bringing this information with you in written form, you will be more likely to provide a more complete history of your medical treatment when you apply for disability. And since the decision on your disability claim will be based on the information found in your records, it will be in your best interest to provide as much information as possible when you apply.
