There are three important things to remember about paperwork and forms that must be supplied to the social security administration in relation to SSD and SSI applications and appeals (reconsideration appeals and disability hearing appeals.
1. No matter what type of form it is (disability report form, work activity questionaire, activities of daily living questionaire, etc), complete it in detail. In other words, the more information the better. Particularly with your work history and medical history, leave no gaps.
2. Submit the form timely. This means, in most cases, completing it as soon as you obtain the form, as soon as it is given to you, or as soon as it is mailed to you. In other words, don't procrastinate and delay your case one bit.
3. Repeat steps one and two.
